WELCOME TO WELLFIELD HEALTH CENTRE

We are pleased to confirm that your registration with the practice is now complete.

You will receive a text message naming your allocated GP. however please be assured that you are welcome to see any GP of your choosing when an appointment is needed. If you do not receive this message, please do not hesitate to call the surgery on 01706 397 600 and a receptionist will be happy to help.

Wellfield Health Centre contributes to the Summary Care Record (SCR) which, with your consent, can be used by authorised healthcare staff providing emergency care anywhere in England. If you would like to have a Summary Care Record you do not need to do anything and one will be created for you, but if you do not want a Summary Care Record please complete the enclosed opt out form and return to the surgery. For further information to help you decide, please read the enclosed factsheet. The surgery also shares your medical record with other health and social care professionals involved in your care via a share for you record. Please visit www.pennineshareforyou.nhs.uk/ for more information. If you would prefer that we didn’t share your record then you can opt out at any time. Please speak to your GP or a member of the reception team.

Wellfield Health Centre also offers the Electronic Prescription Service; please see the enclosed factsheet for more information. If you want to use the service please speak to a member of staff at the surgery or at your preferred pharmacy.

Wellfield Health Centre also offers Health Checks to new patients. This is a full screen which captures information such as family history, weight, waist measurements and a blood pressure check. If you wish to have a new patient health check, please contact the surgery to arrange a 20 minute appointment with one of our Health Care Assistants.
